Our team offers high quality short term housing support to people in West Fife aged 18 and over who hold a council tenancy. We’ll support you to develop skills, to connect with activities and other sources of support in your community, and to gain control in your life. Support can last for up to one year.
What can I expect?
We want to know what matters to you! That’s why we’ll support you to make informed choices about your daily living. Through practical and emotional support, we’ll work with you to create a support plan that reflects the things you want to achieve. We’ll work with you to create a support plan that tells us what you’d like to do with the help of support. The plan will guide your support and is a helpful tool in supporting you to achieve your goals. Regular reviews will help keep track of your progress, give you and your practitioner the chance to discuss your support, and decide any changes. This way, your support is flexible and responsive to your needs.
This can include things like managing your budget, looking after your home, and linking in with other people involved in your life like GPs and social workers. We can support you to connect into local activities, shopping, community networks, and public transport.
How do I access support?
This service is for people aged 18 and over who live in West Fife and who hold a council tenancy. You can access support by contacting us direct. This is called self-referral.
You can also be referred by someone on your behalf like for example a social worker, GP or voluntary organisation.
