Support is funded initially for 12 weeks. Our team will support you to become empowered to make informed choices about your daily living, while the local autoritys ARBD team carry out varioes assessment of your needs. We will support you to gain the skills you need to live independently and meaningfully so you can return to your home or an alternative suitable accomodation that meets your needs. We’ll work together with you to discover your strengths, and will support you to set and achieve your goals one step at a time. Some of team members have lived experience of managing long term conditions and can bring their own insight to support you on your journey.
